Well, after a bit of poking around http://microformats.org/wiki/hcard  
and http://ietf.org/rfc/rfc2426.txt, I can't seem to find anything  
specified. It appears to be undefined in vcard and x.500. So, I'd say  
just but it in the tel element like:

<span class="tel">
	<span class="type">cell</span>: <span class="value">800 555-1212 x  
1234</span
</span>
