Perhaps using both "additional-name" and "title" on them would be good
practice.  As in...

<span class="vcard ">
    <span class="fn n">
        <span class="title additional-name">Mirza</span>
        <span class="given-name">John</span>
        <span title="family-name">Doe</span>
    </span>
</span>

That kind of matches how people use things like that.
